What determines whether endometriosis recurs after surgery?

I will now present a study that focuses on predicting recurrence of endometriosis, in particular identifying risk factors for recurrence after surgery. Endometriosis is a chronic disease that often recurs and can have a serious impact on patients' quality of life. The research seeks to answer what factors can help identify patients with a higher risk of recurrence.

The researchers studied 794 women who had undergone surgery for pelvic pain or pre-existing endometriosis. This was an observational study where patients were followed up after surgery. The study took into account various clinical and surgical characteristics, such as the location of lesions, the presence of adenomyosis, and different types of tissue adhesions.

The analyses identified several important risk factors for recurrence of the disease. Lesions in the intestinal regions, adenomyosis, and the presence of significant adhesions were found to increase the risk of recurrence. Patients with pelvic organ adhesions and bowel involvement were found to have a particularly high risk of endometriosis recurrence.

The researchers stressed that taking these factors into account can help to individualise treatment and long-term management of the disease. They stressed that accurate preoperative diagnosis and identification of risk factors for recurrence could be key to better clinical outcomes. The research also pointed out that adenomyosis, which is often secondary to endometriosis, is also a strong risk factor for recurrence, but also important are high rASRM score, deeply infiltrated disease, bowel involvement, adenomyosis, emergency department visits for pelvic pain, early menarche, multiple previous pregnancies , hypertension and older age. In the surgically confirmed group, based on a 5-point assessment, the RiskSLIM method was able to predict the risk of recurrence with an accuracy of 95.3%.

An important conclusion of the study is that successful treatment of endometriosis requires not only improved surgical technique, but also the early identification and management of risk factors for recurrence. Such a comprehensive approach can help improve patients' quality of life and reduce recurrence.
