Endometriosis and risk of heart disease

Endometriosis, which affects around 10% of women of childbearing age, goes well beyond problems of the reproductive system. Recent research has shown that the disease induces systemic inflammation, oxidative stress and endothelial dysfunction. These processes may affect liver function, alter adipose tissue metabolism and lead to an unhealthy lipid profile, all of which may be associated with an increased risk of cardiovascular disease (CVD) in young women.

To explore this relationship, Dr Havers-Borgersen and colleagues at the University of Copenhagen conducted a large cohort study using data from the Danish National Health Insurance Scheme. By analysing more than 40 years of data, they compared 60 508 women with endometriosis with 242 032 controls, matched 1:4 by age and year of diagnosis. The primary objective was to investigate the incidence of myocardial infarction and stroke, while the secondary objectives were to analyse arrhythmias, heart failure and all-cause mortality.

Women with endometriosis had more co-morbidities and a much higher drug consumption profile than the control group. During the follow-up period, 5.9% of women with endometriosis had a myocardial infarction or stroke compared to 4.9% in the control group (p=0.01). The 40-year cumulative incidence of heart failure was significantly higher in women with endometriosis (9.7%) than in the control group (8.3%) (p<0.001)

The researchers found that women with endometriosis had significantly higher rates of arrhythmias and heart failure compared to controls. They had a 15% higher risk of major cardiovascular events compared to the control group.Interestingly, however, the overall risk of death was lower in women with endometriosis, which they explained by increased medical attention and early intervention due to endometriosis.

This ground-breaking study highlights the need for further research to uncover the mechanisms linking endometriosis and cardiovascular disease. A deeper understanding of these processes will allow clinicians to better support women with this complex condition.
