Endometriosis and autoimmunity

Endometriosis has long been suspected to be more than just a gynaecological problem, but researchers have yet to produce such strong evidence. Painful menstruation, infertility or chronic pelvic pain and other organ-specific symptoms may be caused by much deeper, systemic processes. A recent US study of data from hundreds of thousands of women has now confirmed that endometriosis is linked to a number of autoimmune diseases.

Researchers at Columbia University analysed two huge health insurance databases (MarketScan and Medicaid), which contain data on more than 30 million people each year. They compared more than 332,000 women with endometriosis to 1.2 million healthy controls. The result was clear: women with endometriosis were much more likely to be diagnosed with autoimmune diseases such as rheumatoid arthritis, lupus, multiple sclerosis, Hashimoto's thyroiditis, Sjogren's syndrome or even type 1 diabetes.

Why is this important? Because until now, the link has been suggested by mostly small studies, but now the largest, most comprehensive analysis to date has confirmed the hypothesis. Researchers suggest that common mechanisms may underlie the disease: chronic inflammation, immune imbalances, hormonal-immunological interactions and genetic predisposition. Together, these may explain why endometriosis predisposes to autoimmune diseases, and vice versa.

It is important to understand, however, that this research alone does not conclusively prove that endometriosis causes autoimmune diseases, but only shows a close association. It may also be that patients are being diagnosed earlier because of more frequent medical examinations.

Nevertheless, the message is clear: endometriosis is not just a disease of the pelvis, but a condition that affects the whole body. It is therefore worth paying attention to autoimmune symptoms in the care of affected women and involving doctors from other specialties as soon as possible if necessary.

This study provides a new framework for the picture of endometriosis. It is not just about infertility, menstrual pain or surgery and hormonal treatment, but a systemic disease that also affects the immune system, and understanding this could be key to future treatment and research, and even reform the way endometriosis is treated.
